Set Up School and Medical Authority for a Child in Kinship Care

Write one dated page titled “Care Baseline – [child's name]” on paper or in a document you can print. Include: the child's full legal name and birth date; the date the child came to live with you and the reason in one or two sentences; each parent's current location, custody or legal status, and whether you can reach them; and any court order, case number, or child welfare agency involvement you know of. Keep the wording factual and short — every form in the steps ahead repeats these same facts.
