This guide covers only the presumptive PACT Act pathway: a diagnosed condition on VA's presumptive list, matched to a qualifying service location, filed on VA Form 21-526EZ. It does not cover filing on a direct-service-connection basis (which requires a medical nexus opinion) and it does not cover the appeals process if your claim is denied — those are separate paths.

What you record here — the exact diagnosis wording and your branch of service — is read back by every later step, so get the wording right before moving on.

    To do

    • Look at the diagnosis line (often ICD-10 coded) from your primary care provider, pulmonologist, oncologist, or VA provider.

    • Use the linked VA.gov page to confirm your condition's wording matches a listed presumptive condition for your exposure category.

Enter your diagnosed condition exactly as it is written in your medical record (e.g. "chronic sinusitis", not a summary).
