Advance-directive law is state law. The state where you legally reside supplies the forms and sets the signing rules — not your birthplace, not the state a relative lives in, and not a federal form (there is none). Where you would actually receive care matters just as much: the hospitals and health systems nearest your home are where these papers will be asked for.

The stakes are concrete.
