This step produces a one-page board record: the full names, websites, and phone numbers of your current and destination state boards, your license number, issue and expiration dates, and a discipline line. The trap is names — a board often sits inside a larger department (health, education, regulation), and the licensing division has its own page and phone number.
- Take out your current license document and copy the license number, issue date, expiration date, and the issuing board's exact name.
- Find the destination board: search “[state] [profession] licensing board”, or use your profession's national directory — NCSBN's board list for nurses, your state education department for teachers.
- Open both boards' websites and record the licensing or verification page URL plus the phone number listed for license inquiries.
- Pull your license through your current board's public verification lookup and save the status page (Nursys for RNs and LPNs).
- Add the discipline line: any past or pending action, consent order, or probation — boards ask about it on every application.
- Save the page as a PDF named board-record-[destination-state].pdf.
