  1. Complete the RSTC medical questionnaire
    10 min

This step produces a completed RSTC Medical Statement — the form that decides whether you can train without a physician visit. Every “yes” answer flags a condition that needs a doctor’s sign-off before any in-water training. Do not pay for a course until this form is settled — if a medical issue surfaces later you may forfeit the fee.

    Do

    1. Download the current RSTC Medical Statement from the link below. The form is identical across PADI, SSI, NAUI, and other agencies.
    2. Read each question carefully. The form covers heart and lung conditions, sinus and ear problems, diabetes, asthma, epilepsy, psychiatric conditions, abdominal surgeries, and medications.
    3. Tick “yes” or “no” for every item. An unanswered question is treated as “yes” by most shops.
    4. If every answer is “no”, sign and date the bottom section — you are cleared to proceed. If any answer is “yes”, leave the physician section blank and go to the next step.
