What you need first
You hold a prior VA decision letter on the issues you want to contest, and you can sign in to va.gov with a verified Login.gov or ID.me account. You can reach your DD 214, your service treatment records, and records from your treating providers. A current medical diagnosis exists for each condition at issue. You are willing to pay a private provider if a nexus letter or independent medical opinion is needed, since VA does not reimburse those costs.

- nexus
- The medical link between a current diagnosis and an in-service event or aggravation. It is one of the three elements VA requires for service connection, alongside a current diagnosis and an in-service event; a decision letter that names this element as missing is stating there is no medical opinion connecting the condition to service.
- supplemental claim
- A VA disability claims review filed on VA Form 20-0995 asking VA to reconsider a denied or under-rated issue based on new and relevant evidence that was not already in the claims file when the prior decision was made. Filed within one year of the decision date, it can preserve the original effective date and the back pay tied to it.
- new and relevant evidence
- Evidence that meets two separate tests under 38 CFR 3.2501: it must be new, meaning it was not already part of the claims file when VA made the decision being challenged, and relevant, meaning it tends to prove or disprove an issue in the claim. A document VA already considered does not count as new, even if it is resubmitted.
- increase claim
- A claim filed on VA Form 21-526EZ for a condition that has genuinely worsened since the prior rating decision, where there is no new evidence suggesting the original decision itself was wrong. It is the correct filing instead of a supplemental claim when the only change is that symptoms have gotten worse.
- higher-level review
- A VA disability claims review filed on Form 20-0996 in which a senior reviewer looks again at the evidence already in the claims file, with no new evidence added. It is the correct path when the disagreement is with how VA weighed the existing record rather than with what evidence was in it.
- C-file
- The complete VA claims file for a veteran, containing every document VA considered in reaching a decision, including exam reports and the rating worksheet. It can be requested on VA Form 20-10206 and routinely takes six months or more to arrive.
- VA Form 21-4142
- The Authorization to Disclose Information form that lets VA request records directly from a named treating provider or facility. To be processed it must state the provider's full name and street address and the exact treatment date range, along with the conditions treated; a form missing any of these details is returned or ignored.
- nexus letter
- A written opinion from a treating or independent medical provider stating whether a current condition is linked to an in-service event or aggravation, and explaining the reasoning by citing specific entries in the service and treatment record. A letter that only recites the legal standard without this reasoning carries little weight with VA raters.
